Travis Collins

Travis Michael Collins, age 29, of Columbus, Ohio passed away unexpectedly Thursday, September 26, 2019. He was born November 6, 1989 to parents Tammy Collins and George Hazlett. Travis had an infectious smile, a heart of gold and great sense of humor; you could always count on him for a laugh. Travis was an avid football fan and enjoyed spending his free time watching The Ohio State Buckeyes and The Green Bay Packers. Travis is preceded in death by his Father George Hazlett; Grandmother Margaret Hazlett; and Grandfather Patrick Collins. He is survived by his Mother, Tammy Collins; Brothers, Keith Collins, Anthony Collins, Lucky Collins and Cody Hazlett; Sister, Leshelle Collins; Grandmother, Rose Collins; Grandfather, Roger Hazlett; As well as many aunts, uncles, nieces, nephews, cousins and friends. A memorial service celebrating Travis' life will be held on Saturday, October 12th from 12pm-2pm at Shaw-Davis Funeral Home located at 4341 N. High St, Columbus, OH 43214.
